Students who participate in this program will provide home visits and outreach (phone calls, text messaging, etc.) to high-risk, pregnant women in Camden. They will serve as health coaches for patients of Cooper's Women's Care Center who have a diagnosis of diabetes, gestational diabetes, hypertension, and/or high have BMI. As health coaches, they will provide social support, customized nutrition, exercise, and other education modules, as well as reinforcement of physician messaging and accompaniment to their prenatal visits.
